Request for Returned Material Authorization

Process for requesting a RMA:

  1. Complete the form below.
  2. Agree to the terms (below).
  3. If you desire, you will have the opportunity to upload a photo/file. Click the "Choose File" button, navigate to the desired file.
  4. Subject to the reason for return, an evaluation fee may apply. This fee will be quoted at the time the RMA is issued. A purchase order may be required.
  5. After receipt and review of the request, Dynex will either issue an RMA directly or through one of our distribution partners. If Dynex decides not to provide an RMA, we will contact you directly to explain our decision.

  • I hereby acknowledge that any product authorized for return must be properly packaged, must be sent back complete (not missing any parts), must not have extra parts attached, must be drained of fluid (except water glycol) and must be clean of external debris. Failure to comply may result in the product not being accepted and being sent back at my expense and/or a cleaning fee may be applied. Furthermore, I agree that Dynex/Rivett Inc. is not responsible for any items, not deemed to be part of the original product, against loss, theft, or damage. I understand all items including fittings, hoses, couplings, and or other accessories attached to the original product should be removed prior to return to Dynex/Rivett Inc.
